We're a fast-growing telehealth practice with a 4.9/5 patient rating, and we're looking for a passionate Patient Care Coordinator to join our team and help us deliver an exceptional experience to every patient we serve.

THE ROLE What You'll Do

As a Patient Care Coordinator, you'll be the primary point of contact for our patients — guiding them from intake through their entire weight loss journey with warmth, professionalism, and expert knowledge of our programs.

• Respond to patient inquiries via phone, text, and email
• Conduct onboarding calls and explain program options
• Coordinate with providers and pharmacy partners
• Process refill requests and track patient progress
• Maintain accurate records in our patient management system
• Support patients through side effects and dose adjustments
• Follow up on outstanding medical intake forms
• Escalate clinical concerns to the appropriate provider
